Young Gymnast Approaches Life's Challenges with Determination and Spirit

Bella Tucker, an 8-year-old gymnast on the Phantom Gymnastics team in Hampstead, N.H., came down with a near fatal infection of Streptococcus Pneumonia Sepsis with DIC (disseminated intravascular coagulation) on Easter Sunday. Doctors had little hope for this third grader's survival, but after five days in a coma, she miraculously survived.

Unfortunately, the infection caused extensive tissue damage, and on April 27, 2010, Bella underwent quadruple amputation surgery and is currently facing an extremely long and hard recovery at Children's Hospital in Boston with extensive rehabilitation to follow as she learns to live life without her limbs.

Preparing for the 2010-2011 Season

Registration for the 2010 competitive season will begin early in June 2010. All membership and sanction application forms are available here. All forms and applications will also be emailed to all USA Gymnastics Member Clubs and non-member clubs.

Please note that USA Gymnastics no longer mails application forms to clubs. All membership application forms and information are available on the USA Gymnastics website. Online registration will also be available for all member types.

Please see below for a few hints and reminders to prepare for the new season.

  • Destroy all unused forms from previous season. USA Gymnastics is unable to process or register any member application that is submitted on an expired form.
  • Remember that both athletes (and parents if gymnast is under 18 years of age) MUST sign the Athlete and Introductory Athlete application form even if the transaction is completed online.
  • You are to keep the completed and signed form for all of your athlete members on file in your club. Insurance benefits may not be paid out if this form cannot be supplied. You should plan to store the completed forms for a minimum of 7 years. Please consult with your attorney/legal council to determine the proper length of time to save and store these membership forms.
  • Athletes Level 4 and above must obtain a Regular ($49) Athlete membership.
  • Level 1 athletes, Women's prep-optional and Group Gymnastics Levels A&B may obtain an Introductory ($20) membership.
  • Please contact Member Services to transfer, upgrade or adjust any member records prior to registering online. Member Services may be contacted at 800-345-4719 or via email at membership@usa-gymnastics.org.
  • Professional membership must be held by all coaches and judges ages 18 and older. Junior Professional membership must be held by all coaches and judges ages 16 and 17. Membership must be current and active. Pending members are not eligible for floor access at USA Gymnastics sanctioned events.
  • Reminder: Background check screening is mandatory for all Introductory Coach members. Please visit the USA Gymnastics website at www.usa-gymnastics.org for more information regarding the background check screening.
  • Register athletes as soon as possible so that they receive a full year of benefits. Remember USA Gymnastics magazine subscriptions begin with the Sept./Oct. issue. Parents often call upset when they learn that even though they turned the form and registration fees into a club in the summer, that the club did not file the registration until later and their membership is not active until that time.
  • Don't forget to register your athletes online and save $5.00 for each registrant.
Please visit the USA Gymnastics website and watch for the Member Services E-Newsletter, for the most up-to-date and current membership information.

We continue to thank you for your support and wish you and your club the most successful 2010-2011 season!

Safety Matters: Athlete Application Forms are a Safety Issue

As we prepare to open the 2010 season, we would like to remind Professional members, gym owners and program managers why it's important to obtain and store USA Gymnastics membership application forms for all athletes in your programs.

  • You should have signed a hard copy of the Athlete membership application form for all athletes in the current season regardless of whether you've registered them online. It is imperative that you keep these forms on record with your other business and tax records and for a similar duration. The signed Athlete membership form, as well as your gym's waiver and release forms, are a part of your gym's comprehensive risk management program. In some cases, they may be the only documentation you have that the athlete and their parent(s) were made aware, and acknowledged, the risks of gymnastics if the athlete should be injured and seek legal action.

  • We reserve the right to randomly audit gymnastics clubs to confirm possession of Athlete membership forms.. By using the online membership registration at www.usa-gymnastics.org, you have acknowledged that you have hard copies of each athlete form on file. If you register athletes online but don't have the forms, you have used the online system fraudulently and your access may be suspended.

  • USA Gymnastics suggests all gymnastics businesses seek legal counsel regarding risk management issues and policies particular to your state.

USA Gymnastics is a non-for-profit organization which is the sole national governing body (NGB) for the sport of gymnastics in the United States.
