2013 P&G Gymnastics Championships Registration Packet - Women's Artistic Gymnastics

Congratulations for qualifying to the 2013 P&G Gymnastics Championships. This event will be held in Hartford, Connecticut, from August 15-18 at the XL Center. The purpose of this notification is to provide you with information regarding forms, hotel reservations, and tickets for family members and additional guests. The Participant Information Packet, including the final daily schedule, will be available upon processing at the USA Gymnastics Registration Office.

  • Technical packet

  • Athlete medical forms: Pre-participation Physical Evaluation, Authorization (HIPPA) Form, Authorization to Obtain & Disclose Information, Consent to Treatment and Verification of Other Insurance Form, must be completed by Monday, July 29, 2013, and returned to: Rachel Brazo, Director of Program Administration. You may return the forms by email rbrazo@usagym.org or fax 317/237-5069. You may complete the forms online, print them out for signatures and return them via fax or email. You will not receive an event credential onsite until all the appropriate forms are completed.

  • P&G Championships Participant Information Form: In addition to registering for the P&G Championships online all athletes must also fill out this registration/information form by Monday, July 29, 2013, and returned to: Rachel Brazo, Director of Program Administration. You may return the forms by email rbrazo@usagym.org or fax 317/237-5069. You may complete the forms online, print them out for signatures and return them via fax or email. You will not receive an event credential onsite until all the appropriate forms are completed.

  • Additional Coaches: Each athlete is eligible to bring one coach, with a maximum of two coaches per club. Additional coaches credential requests must be approved by Rachel Brazo, Director of Program Administration. An Additional Coaches Credential Request Form along with payment must be submitted to: Rachel Brazo, Director of Program Administration, by fax 317-237-5069 or email rbrazo@usagym.org.

  • Each athlete must complete an online athlete biography. If you haven't filled out or updated your bio before, instructions can be found here. Current National Team members need to go online to verify their current information on the USA Gymnastics website. Bios must be completed no later than July 17, 2013. Also, a headshot photo of each participant will be taken when you register at the USA Gymnastics Registration Office. If you have questions, please contact Matt Steinke at msteinke@usagym.org.
  • Event Schedule

  • Hotel reservations should be made immediately with NTS. You may complete your request online at www.ntssportstravel.com or by calling 1-888-603-8747.

  • Event Tickets: As an athlete or club/team you will receive two (2) complimentary tickets to the session(s) that you will be competing. These complimentary tickets are located in an athlete/club ticket block (Silver or Bronze seating) and will be distributed at registration unless additional tickets are purchased then all tickets will be placed at Will Call. Clubs/teams of multiple athletes in one session are only entitled to two (2) tickets. Please e-mail Jackie Magnuson at tickets@usagym.org with any questions.
